8. Planning:
a. Prior to requesting the written Soil Disturbance Permit and/or an Excavation Permit, the requesting supervisor shall inspect the work area and determine the soil type via a competent person.
b. Identification of the location of all underground utilities that may be impacted by the soil disturbance activity shall be conducted prior to the excavating of any soil
i. Blue stakes or the State’s designated locating service shall be utilized to identify all public utilities. The blue stakes reference number needs to be available to add to the soil disturbance permit.
ii. All nonpublic-ran utilities shall be identified
c. The Okland utility plot plan shall also be referenced.
d. The location of isolation valves/switches for utilities in close proximity to the soil disturbance operation must be located and identified by all workers involved in the soil disturbance work.
e. An emergency action plan shall be developed for the operation specific to a utility strike. The plan shall be conveyed to all workers involved in operation.
