4.0 FALL EXPOSURE CONTROLS
Are these controls in place?
4.1 TRAINED BY COMPETENT PERSON
Competent person trained workers using in fall protection
4.2 SUPERVISION BY COMPETENT PERSON
Qualified person supervised design, installation, and use of fall protection
4.3 IMPALEMENT HAZARDS
No impalement hazards (e.g. poorly staged rebar, uncapped rebar)
4.4 FALL PROTECTION ABOVE 6FT
Use of fall protection for all workers working at 6ft or higher (excluding ladder)
4.5 EQUIPMENT CONDITION
Fall protection on lanyard in good condition (no cuts, tears, stitching gaps, chemical/UV damage, metal deformation or corrosion, impact indicators)
4.6 WEARING HARNESS
Harness and lanyard being worn properly
4.7 ANCHOR POINT
Anchor point selected is acceptable
4.8 HOLE COVERS ARE LABELED
Hole covers are clearly labeled with "HOLE" or by clear visual means (e.g. bright orange engineered cover)
4.9 HOLE COVERS ARE SECURED
Hole covers are cleated and/or secured in place
4.10 HOLE DUST PROTECTION
Visqueen, drywall, etc. used to temporarily protect shaft from dust or air flow; is marked "HOLE" and protected by guardrails
4.11 GUARDRAIL UP & SECURE
Guardrail up and secure within the safety zone of the hazard and meets top/midrail heights & capacities
4.12 TOE BOARDS IN PLACE
Toe boards are in place
4.13 STANCHIONS ARE SECURE AND EVENLY SPACED
Stanchions are secure and evenly spaced
4.14 CABLE SYSTEM TIGHT
Cable system is tight with proper deflection
4.15 LADDER USE
Ladders being used correctly (per Okland checklist)
4.16 LADDERS WORKING CONDITION
Ladders are in good working condition (per Okland checklist)
4.17 WALKWAYS WITH CHANGE IN ELEVATION
Walkways with a change in elevation of 19" or more has stairway, ladder, or ramp
4.18 TEMP LADDERS
Minimum of 1 temp or permanent stair operational for structures of 3-stories or more
4.19 TEMP STAIR SHALL BE FILLED
Temporary stairway and landings shall be filled with wood or other solid material to top edge of pan
