5.0 SCAFFOLDS CONTROLS
Are these controls in place?
5.1 ENGINEERED SCAFFOLD WITH WRAPPING
Engineered design of scaffold is confirmed when over 120' in height or if any wrapping is used (could create a sail)
5.2 COMPETENT PERSON SUPERVISION
Competent person supervised erection, alteration, moving, or dismantling
5.3 QUALIFIED PERSON TRAINING EMPLOYEES
Qualified person trained employees working on the scaffold
5.4 GREEN SCAFFOLD TAG SINGED
Green scaffold tag is signed prior to each shift by competent person
5.5 WORKERS FOLLOWING CONTROL - YELLOW FLAG
Workers following all controls if scaffold has yellow tag & is signed prior to each shift by a competent person
5.6 SCAFFOLD IS SECURED TO STRUCTURE
Scaffold is secured to structure when height is 4X the width of the base (protection from anticipated wind loads)
5.7 GATES ARE INSTALLED
Gates are installed on the scaffold access/egress points
5.8 LADDERS ARE PROPERLY INSTALLED
Ladders are properly installed on the scaffolding
5.9 RAILS AND TOE BOARDS IN PLACE
Horizontal top rail, horizontal midrail, and toe boards in place
5.10 NO CROSS BRACING AS RAIL
No cross bracing used as guardrail or midrail
5.11 CROSS BRACING AND PINS IN PLACE
Scaffold cross bracing and pins in place on both sides of scaffolding
5.12 PLANKS ARE MARKED AS ANSI RATED
Planks are marked as ANSI rated
5.13 NO LARGE GAPS OR CUTS IN PLANKS
No large gaps or cuts in planks (less than 11")
5.14 ADDITIONAL PROTECTION
As needed, additional protection in place for falling objects; screens, nets, canopy structure, or barricades
5.15 CASTER/WHEELS USE
If casters/wheels are used, they are locked if workers are on the scaffold
