Potential hazards are controlled/mitigated through the use of LOTO, isolation, and/or purging
8.5 ATMOSPHERIC MONITORING
Atmospheric monitoring is conducted prior to entry and additionally, as appropriate (continuous vs. periodic)
8.6 VENTILATION ELIMINATED HAZARDS
Where continuous forced air ventilation is used to mitigate a hazardous atmosphere, no employee shall enter the space until the ventilation has eliminated identified hazards
8.7 RESCUE PLAN & EQUIP.
Rescue plan and equipment is adequate for specific entry hazards
8.8 LOCATION OF PERMIT
Location of permit required confined spaces marked and labeled
8.9 CONFINED SPACE IS BARRICADED
Confined space is barricaded and demarcated when the potential for exposure exists
8.10 ATTENDANT IN PLACE
Attendant (e.g. hole watch) in place and understands responsibilities
